چکیده
(CHEST 1999; 115:267–268) A 50-year-old woman develops a gradual deterioration in oxygenation shortly after single lung transplantation. This patient, with alpha-1-antitrypsin deficiency, undergoes right lung transplantation for emphysema. The surgery performed through a right thoracotomy is uncomplicated and the patient is transferred to the ICU for recovery. Shortly after arrival, the nurses notice a gradual but persistent deterioration in her oxygenation. Pulmonary artery balloon flotation catheter readings show a pulmonary artery balloon occlusion pressure of 10 mm Hg. The chest tubes are draining serosanguinous fluid without gross blood. A chest roentgenogram is obtained (Figure 1). The most likely cause of the patient’s problem is:

Extracorporeal membrane oxygenation after living-related liver transplant.
Various types of extracorporeal membrane oxygenation methods have been used in liver transplant operations. The main indications are portopulmonary or hepatopulmonary syndromes and other cardiorespiratory failure syndromes that are refractory to conventional therapy. There is little literature available about extracorporeal membrane oxygenation, especially after liver transplant. We describe ou...

متن کاملExtracorporeal membrane oxygenation as a bridge to lung transplantation.
Idiopathic pulmonary artery hypertension (IPAH) is a progressive disease with a dismal prognosis and lung transplantation is often the only option for patients, who do not respond to pharmacological therapy. We report the use of an extracorporeal membrane oxygenation (ECMO) system in a 49-year-old woman with primary pulmonary hypertension, previously liver transplanted. The patient, listed for ...

متن کاملTransplantation of lungs after ex vivo reconditioning in a patient on semi-elective long-term veno-arterial extracorporeal life support.
We present the case of a 41-year old patient suffering from end-stage pulmonary hypertension secondary to veno-occlusive disease who underwent implantation of a veno-arterial extracorporeal membrane oxygenator as a bridge to lung transplantation (LTx) due to significant deterioration of myocardial pump and liver function. After 33 days on support, lungs with extended donor criteria were offered...
